What information we collect

When you complete the FANOS Team & Collaborator Information Survey, we collect:

• Your full legal name and preferred name
• Email address and phone number
• Your FANOS role, title, and relationship to FANOS / Alchemizing Stories
• Organization or school affiliation, if provided
• For participants under age 18: parent or guardian contact information
• Information about which agreements apply to your role
• Any signed agreement documents you upload
• Communication preferences and time zone
• Conflict-of-interest disclosures
• Privacy, accessibility, or communication notes you provide
• Your typed acknowledgment and date of completion
• Technical information: IP address and browser identifier (for record integrity)

Why we collect it

FANOS uses this information to:

• Manage your role and agreements as a FANOS team member or collaborator
• Maintain required agreement records for legal and operational purposes
• Communicate with you about your role
• Protect the privacy and confidentiality of everyone connected to FANOS and Alchemizing Stories
• Support FANOS business continuity

How we store it

Submissions are stored in a secure server-side data store accessible only to authorized FANOS administrators. Uploaded files are stored on the server with unique, non-guessable filenames. The database is not exposed to the public internet.

This portal does not use third-party analytics, tracking pixels, or advertising tools.

Who can access your information

Access to your submission is limited to FANOS leadership and administrators who have a legitimate operational need. FANOS does not sell, share, or trade your personal information with third parties, except as required by law.

Your rights and data requests

To request access to, correction of, or deletion of your submission record, please email contact@fanosconsulting.com with your Record ID (if available) and a description of your request. FANOS will respond in a reasonable timeframe.

Note: certain records may need to be retained for legal, compliance, or operational purposes even after a deletion request.

Minors

If the participant is under age 18, a parent or legal guardian must participate in the submission process. Guardian information is collected solely for the purpose of fulfilling the guardian acknowledgment requirement in the FANOS Agreement (Page 21).
